IN Unit 4 you will learn to do the following:
- Complete a physical therapy history interview and examination with clinical instructor supervision
- Apply didactic knowledge and skills to a physical therapy evaluation in a proficient manner
- Identify and categorize patients' impairments accordingly using ICF language
- Develop short-term goals and long-term goals that are patient-centered and evidence-based following a physical therapy evaluation
- Design interventions appropriate to each impairment identified in the plan of care following a physical therapy evaluation
- Adapt delivery of physical therapy care for a patient with a socioeconomic hardship or cultural diversity in 3 or more ways
- Identify a minimum of 3 areas in which your clinical performance meets expectations and 3 areas in which your performance needs improvement using the APTA CPI
- Compose 3 or more statements of constructive feedback for your clinical instructor(s) on his/her/their clinical instruction skills using the PTSE tool
- Self evaluate your professional behavior as an adult learner and develop 3 goals and 3 strategies to improve your weaker rated abilities using the Generic Abilities assessment tool
*Note: You may notice that there are many more objectives that you will meet within this unit. I would like to make sure that you are aware that this unit is spread over a 2-week time frame, therefore more objectives will be covered.
1. Create a case scenario demonstrating socioeconomic hardship or cultural diversity.
As you are noticing during the first 3 weeks of your clinical internship, no two patients are the same. Two factors that can greatly impact your physical therapy intervention and plan of care is your patient's socioeconomic status and cultural characteristics. For example, if your patient is currently experiencing a financial hardship, it is not helpful to recommend your patient purchase something to complete his/her home exercise program. Rather, it is recommended that you instruct your patient to use something that they possess at home or instruct isotonic contractions to complete their home exercises program, if necessary.
For this assignment you are to write a case scenario demonstrating a socioeconomic hardship or cultural diversity case that you experienced with one of your patients. At the conclusion of your case scenario, list 3 different techniques that you used or could use to meet your patient's needs ethically and respectfully. Your case scenario post should be < 250 words. Be sure to exclude any and all patient identifiers to avoid any HIPAA violations to protect patient confidentiality. Post your case scenario on our class Facebook page. You will earn participation points for completing this assignment. You should post this case scenario on the class Facebook page before August 23, 2015.

2. Major Assignment 3: Complete a Physical Therapy Evaluation.
This major assignment is exactly what the title says - complete a physical therapy initial evaluation. In this assignment you will be taking the lead in completing your first physical therapy initial evaluation of a non-complex patient (deemed non-complex by your clinical instructor). A non-complex patient is a patient who is presenting for physical therapy evaluation and treatment for one specific impairment, and is generally high-functioning in general society and typically independent in performing most activities of daily living.
For this assignment, you will take the lead on performing all components of the physical therapy initial evaluation to include the examination, evaluation (including diagnosis and prognosis), and plan of care (including writing goals and physical therapy treatment intervention). Remember to use International Classification of Functioning, Disability and Health (IFC) language throughout your evaluation. Although ICF language should not be new to you, I provided a resource about ICF language below.
You will present your physical therapy initial evaluation in the form of a presentation using Prezi.com. Prezi.com is the same software application that you used in Professional Issues class to complete your professional issues presentations. Use your same log-in information to complete this initial evaluation presentation. Your physical therapy initial evaluation project will be graded by the rubric attached below.

3. Major Assignment 4: Complete All Final Evaluations.
This is the last major assignment, whew! You made it!
For this assignment you will be completing 3 different evaluation forms. These forms should demonstrate lots of thought and self-reflection. I will describe each form separately.
1. American Physical Therapy (APTA) Clinical Performance Instrument (CPI).
This is quite a lengthy tool that you will use to self-assess your clinical performance in the clinical setting. This tool is web-based and requires you to complete a training session in order to use this tool properly. To complete the APTA CPI training, go to the APTA Learning Center at http://learningcenter.apta.org/free_membercourses.aspx. You do not need to be an APTA member to complete this training, which means that your clinical instructor does not need to be an APTA member to use the CPI. Once on the APTA Learning Center page, find a complete the Online: 2013 PT CPI course. This course is free to complete. The course will take approximately 45 minutes to 2 hours to complete from start to finish. There is a test at the end of the course to ensure this instrument's validity and reliability. Once you pass the test, you will then be able to log on the the APTA CPI web-based tool at https://cpi2.amsapps.com/user_session/new. I have provided a paper-based copy of the APTA CPI below for you to print and carry with you to your clinical internship to jot down notes as you have time available throughout the day.
This instrument is comprised of 18 performance criteria. For each criteria you will rate your clinical performance using a continuum of 5 anchors ranging from Beginner Performance to Beyond Entry-Level Performance. For each criterion you must list 2-3 specific performance examples of how you demonstrated that criterion well and at least 1 example of your performance that needs improvement in that criterion. In the Summative Final Comments section, you will list 3 examples under each section, "Areas of Strength", "Areas for Further Development", and "Recommendations".
Your clinical instructor will also complete this same evaluation of your clinical performance. For this first clinical internship, your clinical instructor must have scored you between "Beginner performance" and " Advanced Beginner Performance" with no significant red flags marked in order for you to pass this course. If a significant red flag is marked on the CPI, you could be as risk for failing this internship and would indicate the need for a meeting between the student, clinical instructor and course instructor/Director of Education. This APTA CPI will count for 100 possible points for Major Assignment 4.

2. Physical Therapist Student Evaluation Form (PTSE).
This form is an evaluation that that student completes on the clinical instructor and the clinical facility. This form allows the student the opportunity to give feedback on his/her clinical instructor and for the clinical instructor to receive feedback from the student on the clinical facility.
You must complete every item on this form to receive all points possible for this assignment.

The Generic Abilities assessment is an abilities-based assessment consisting of 10 professional behaviors that represent behaviors essential for clinical success. To complete this assessment, download and print this assessment form below. Then, rate your current performance on the continuum rating scale ranging from 1-7. Then, for each behavior list 2 examples of behaviors you have performed or possess that demonstrates this behavior. You will earn 1 point for each behavior that you list for the 10 professional behaviors. At the end of the assessment, you should create and list 3 SMART goals (Specific, Measurable, Achievable, Realistic and Timely) that would help you improve your overall Generic Ability ratings to become a more successful clinician. Then design and list 2-3 strategies that you could do to help you in achieving the above goals. You will receive 1 point for each goal and strategy for a total of 5 points. The grand total of possible points for this assessment is 25 points. You can submit this completed assessment by scanning and emailing to me or faxing the completed form to (901) 448-1411.
